We bring team-based primary care directly to patients who reside in assisted living, independent living, memory care and group residential communities.
We also partner with Minnesota health plans to offer care coordination to their members — helping them understand benefits, arrange care and remove barriers that stand in the way of better health and independence.
We also offer values-aligned primary care practices a launchpad into value-based care proven to help deliver better patient outcomes and practice growth.
