PACE Southeast Michigan is a unique health plan, providing home and community-based care to help nursing-home eligible adults aged 55 years or older with chronic illnesses remain independent in their own homes. Our expert team works with our participants to develop personalized care plans that include social, medical and other services.
We provide access to a full continuum of preventive, primary, acute and long-term care services 24/7, 365 days a year in our day health centers across Southeast Michigan and in the home. By aging in place, older adults have more hope, vitality, joy and an overall higher quality of life.
PACE (Program of All-Inclusive Care for the Elderly) is a national program funded by Medicare and Medicaid. Founded in 1994, PACE Southeast Michigan is a non-profit organization with nine centers located in Southfield, Dearborn, Westland, Eastpointe, Sterling Heights, Clinton Township, Pontiac, and two centers in Detroit.
